Community Connector Program

The Moorditj Koort Community Connector Program aims to engage, inform, and support our mob through the NDIS pathway process.

What is the NDIS?

The NDIS can provide our mob who have a disability with information, connect them to services in the community, and give them details about what support is provided under the National Disability Insurance Scheme (NDIS).

The NDIS supports our mob who have a permanent or significant disability that affects their ability to take part in everyday activities and live an ordinary life.

  • Support to achieve greater independence
  • Support to be more involved in your community
  • Support in gaining employment
  • Support with improved well-being
  • Support to children with a permanent or significant disability

(Children aged 0–6 years and children over 7 years have different NDIS support “pathways”)

Guidance By providing guidance and advice around what the NDIS is
Info Sessions Delivering information sessions and yarning circles to help you understand the NDIS and how to access it
Support Types Supporting you in identifying the types of support that best suit your disability
Forms Assisting with the completion of NDIS Access Request Forms (ARF)
Meetings Attending planning meetings with you
Clarity Providing clarity and understanding around any concerns or questions you may have about the NDIS
